Annual Permission and General Release Form (Minors)

This annual permission and general release form is required for minors participating in parish religious education and youth activities. A custodial parent or legal guardian must review, complete, sign, and return the form to the parish.

The form includes participation permission, emergency contacts, medical information, emergency treatment consent, and a liability waiver. It remains effective for twelve months from the date signed, unless replaced sooner by a new permission form or equivalent.

Media permission is optional. Parents or guardians may decline the media release by marking the checkbox provided. Declining does not prevent their child from participating in activities.
