Parish Drivers Form

Everyone who drives on behalf of one of our churches must complete and submit this form so it is on file with the parish before driving.

This includes driving a parish vehicle, transporting people for parish activities, or simply running an errand for the parish in your personal vehicle. Even a request such as “Could you go to Walmart and pick up some snacks?” falls under this requirement.

(If an accident occurs while you are driving on parish business, a claim could involve the parish as well as the driver. Having the required driver information on file helps the parish manage that responsibility, even for a short errand.)
